      【】目的 通过对60例平脉及60例弦脉的正常成人双侧颈总动脉、桡动脉、足背动脉瞬时波强(WI)各参数的探测及分析,探讨正常成人生理性弦脉的颈总动脉、桡动脉、足背动脉WI参数及脉图特点。分析生理性弦脉的WI脉图参数的特征性变化。方法 应用ALOKA公司Prosound α7彩色多普勒超声诊断仪对60例弦脉及60例平脉的正常成人双侧颈总动脉、桡动脉及足背动脉的瞬时波强WI相关指标进行检测、分析。结果 生理性弦脉组的年龄、BMI、收缩压、舒张压、血总胆固醇均较平脉组高,两组比较差异有显著统计学意义(P<0.01)。生理性弦脉组颈总动脉的NA、β、EP、AI、PWVβ均较平脉组增高,AC较平脉组低,差异有显著统计学意义(P<0.01)。生理性弦脉组桡动脉及足背动脉的NA值均较平脉组高,差异有显著统计学意义(P<0.01);AI值均较平脉组高,差异有统计学意义(P<0.05)。生理性弦脉组受试者的年龄、血压、血总胆固醇、甘油三酯及体重指数均与颈总动脉、桡动脉及足背动脉WI技术的多项反映血管弹性及外周阻力的参数有显著相关性(P<0.01)。结论 生理性弦脉组颈总动脉的血管弹性较平脉组降低,外周血管的血流阻力增高,生理性弦脉组桡动脉及足背动脉的外周血管血流阻力、血管弹性均较平脉组增高,差异有显著统计学意义。因此,可以看出,生理性弦脉是人体动脉血管弹性减退,外周中小动脉所造成的外周阻力增高的一种生理状态。而血压、血脂、年龄、BMI等的增高与生理性弦脉的形成密切相关。WI波形显示生理性弦脉组的NA波较平脉组加深。生理性弦脉可通过WI技术的各项参数及波形客观体现。

      【】Objective To analyze the characteristic changes of WI pulse parameters of physiological taut pulse by detecting and analyzing the WI parameters of bilateral common carotid artery, radial artery and dorsal pedal artery in 60 normal adults with normal pulse and 60 normal adults with taut pulse. Methods The WI related indexes of bilateral common carotid artery, radial artery and dorsal pedal artery in 60 cases of taut pulse and 60 cases of normal pulse of normal adults were detected and analyzed by using Prosound α7 color Doppler ultrasound diagnostic instrument of ALOKA company. Results The testing data shows the age, BMI, systolic blood pressure, diastolic blood pressure and total blood cholesterol of the physiological taut pulse group were higher than those of the normal pulse group, and there was significant difference between the two groups (P < 0.01). The NA, β , EP, AI and PWVβ of common carotid artery in the physiological taut pulse group were higher than those in the normal pulse group, while AC was lower than that in the normal pulse group, with significant difference (P < 0.01). The NA values of radial artery and dorsal pedal artery in the physiological taut pulse group were higher than those in the normal pulse group (P < 0.01), and AI values were higher than those in the normal pulse group (P < 0.05). The age, blood pressure, total cholesterol, triglyceride and body mass index of the subjects in the physiological taut pulse group were significantly correlated with the parameters reflecting vascular elasticity and peripheral resistance of common carotid artery, radial artery and dorsal pedal artery WI techniques (P < 0.01). Conclusion The elasticity of common carotid artery in the physiological taut pulse group was lower than that in the normal pulse group, and the resistance of peripheral blood vessel was higher. The resistance and elasticity of peripheral blood vessel in the radial and dorsal pedal arteries in the physiological taut pulse group were higher than those in the normal pulse group, with significant difference. Therefore, it can be say that the physiological taut pulse is a physiological state in which the elasticity of human arteries decreases and the peripheral resistance caused by peripheral arteries increases. The increase of blood pressure, blood lipid, age and BMI is closely related to the formation of physiological taut pulse. The WI waveform showed that the NA wave in the physiological taut pulse was deeper than that in the normal pulse group. Physiological taut pulse can be objectively reflected by the parameters and waveforms of WI technology.
